It works over the local network or over the internet and looks very impressive, you can get full instant access to all your content without having to sync to the PC.
You can also browse the Media Center EPG, schedule recordings and with an Apple TV you can use Remote Potato HD for the iPad to send recorded TV directly to an Apple TV so effectively you can turn an Apple TV into a Windows Media Center Extender.
